Residence Hall Cable Service Change Request
Please fill out the information on this form and select the appropriate change to your cable service: Cancellation, Modification, Re-Activation.

Notice to those living in doubles, triples, quads, or other living arrangements where there are multiple residents per room: ALL residents of a room wishing to cancel, re-activiate, or modify the cable billing must agree to such changes.

Service Change Requested (choose one)*

If you are in a single/have no roommates, you can only cancel/re-activate your cable. Billing modification is not an option.

If you are in a room that has multiple residents, you can cancel cable, re-activiate cable, or modify cable billing for the entire room. This will affect ALL of your roommates.

Cable Cancellation

To avoid charges, submit this form by the first Friday after classes begin. Forms received after the deadline are subject to prorated charges.

Re-Activation of Cable

Residents who previously canceled their cable can re-activate it for a $25.00 re-activation fee in addition to the normal cable charges.

Modification of Billing

All the residents of a room agree to exclude one or more of the roommates from billing responsibility. The normal cable fees will be divided among the remaining roommates. The person(s) being excluded cannot submit this form.


Resident and/or Roommate Information

If you are in a single/have no roommates, please fill in your name and UMID below. Nothing else is required.

If you are in a room that has multiple residents and:

  • You're cancelling cable for your room- fill in your name and UMID and the name and uniquename of your roommate(s). Ignore the 'exempt' and 'pay equal' options.

  • You're reactivating cable for your room- fill in your name and UMID and the name and uniquename of your roommate(s). Ignore the 'exempt' and 'pay equal' options, UNLESS there is a mutual agreement between ALL roommates to exclude other roommate(s) from charges. In this case, select the “pay equal” or ”exempt” options for each person. (Remember, only a person who is NOT exempt can submit this form).

  • You're modifying billing -fill out your name and UMID (remember only a person NOT being excluded from paying can fill out this form) and the name and uniquename of your roommate(s). You must fill out the 'exempt' and 'pay equal' options for each roommate.

  • Submit only one form per room, NOT one from each roommate.

Agreement

By typing the word "Agree" below the person submitting this cable form agrees that they desire to cancel/re-activate or modify the cable for their room and that, if applicable, all other roommates have been informed and consent to this cable modification.
